OCD Storytelling for Everyone
OCD Storytelling is NOT about telling stories describing OCD. Rather, it is about telling stories about life, and then applying the lessons embedded in those stories towards one's understanding of OCD, its impact, and its treatment. Dr. Allen Weg will read stories to participants, and then ask them to share their perceptions of how each story represents certain aspects of their personal experience of the disorder and/or its treatment. This year's stories will focus on the themes of Trust and Acceptance. We encourage (but do not require) participants to join Dr. Weg in coming dressed in bedtime clothing. We are looking to create a warm, cozy and intimate environment that at once will be fun, educational and compelling, and will serve to help build a shared sense of community. Fuzzy slippers are optional.

Commonly Misunderstood OCD Symptoms: What OCD Is and Isn't

OCD wears many hats!  It is a highly complex disorder with many different types of obsessions and compulsions. Most clinicians, families, and the general public know the more common content areas of OCD, such as contamination and arranging obsessions. Yet, some symptoms are puzzling and knowing when they do or do not fall under the OCD umbrella can be quite challenging. This presentation will provide examples of symptoms of OCD that may easily be confused with other diagnoses. We will help distinguish the OCD patterns from other difficulties, including behavioral concerns, eating restrictions, suicidal thoughts, aggressive tendencies, body focused repetitive behaviors, tics, and interests or preferences.

Reflecting on BDD: Recent Findings and Implications for Treatment

This presentation features four recent findings on the psychopathology and treatment of BDD. First, Ms. Weingarden will present data examining general shame and body shame as predictors of BDD severity and outcomes. Second, Dr. Cougle will share his findings on "not-just-right-experiences" (uncomfortable feelings of incompleteness, imperfection, or perceptions that the environment is not as it should be), and discuss their role in BDD. Third, Dr. Phillips will present data on motives for drug use in BDD. Lastly, Dr. Wilhelm will present findings on mechanisms underlying symptom change in cognitive-behavioral therapy for BDD. Dr. Fang will moderate the discussion and draw key themes across these studies to advance our understanding of BDD mechanisms and treatment.

50 Years of Medically-Based Treatment in OCD: Past, Present and Future

It has been nearly 50 years since Lopez-Ibor published the clinical observation that clomipramine appeared effective in OCD. In 1976, Yaryura-Tobias hypothesized that clomipramine’s effects on serotonin were central to its efficacy in OCD. In 1986, the IOCDF was formed in the context of large-scale clinical trials of clomipramine and other randomized controlled studies of what was then a new generation of SSRIs, such as fluvoxamine. Given that this year marks the 30th Anniversary of the IOCDF, this talk will reflect on what we have learned about medication treatment for OCD and chart a path to the future. The number of available research-proven options are few, and thus the panel of experts will focus mostly on strategies for future drug and device development.

Exposure Therapy for BDD: An Interactive Workshop for People with BDD, their Families and Clinicians

Throughout this conference weekend, attendees will be offered many opportunities to learn about the treatment of choice for OCD: Exposure and Response Prevention (ERP). Challenging? Anxiety provoking? Stressful? Yes, those words are often used to describe the process, but so are rewarding, life-changing, and transformative. In this workshop, the role of exposure therapy will be discussed specifically as it relates to the treatment of BDD. While the presenters will provide a brief overview of how it works, the primary focus will be on experiential learning. Participants will be strongly encouraged to participate, whether they have BDD or are helping someone who does. This will all take place in an environment that is both supportive and respectful.

50 Years of Exposure and Response Prevention for OCD: Past, Present, and Future

It has been 50 years since the first study on exposure and response prevention (ERP) treatment for OCD was published. Since that time, OCD has been transformed from an unmanageable problem to a largely treatable condition. This panel assembles experts in the field of ERP for OCD to discuss developments in this treatment approach over the past 50 years. Panelists will reflect on the most significant contributions to the field as it has evolved, the impact of recent advances and findings in the field (e.g., inhibitory learning approaches), and their predictions for what the next 50 years will hold regarding ERP and the treatment of OCD. Audience participation and questions will be encouraged.

Mind Washing: Mastering the Unique Challenges of Mental Rituals

Many OCD sufferers struggle in silence because their compulsions are silent or covert. This can lead to feeling isolated even within the OCD community, to misdiagnosis, and inadequate treatment. Understanding the role mental rituals play in all forms of OCD can be the key to accessing and using the right CBT tools. This panel consists of OCD specialist therapists who will discuss the research and clinical applications for addressing mental rituals, as well as personal reflections from those who have struggled and developed mastery over those rituals. Several common mental rituals will be identified and explained, and specific treatment interventions will be described, followed by a Q&A session.

Regaining Your Balance When Slipping, Sliding and Backsliding: How to Create an OCD Maintenance/Recovery Plan

After the challenging work of ERP, you want to be done. But, you will slip – slipping is a normal process, and it happens for any behavior one tries to change (e.g., diets, exercise programs, smoking cessation, etc.). If slipping meant OCD returned in full force and propelled you back to dysfunction, that would be devastating. But, this isn't the case. The trick is early detection of warning signs and quick response to slips. In this interactive workshop attendees, will identify the triggers leading to slips.  Roleplays will help them learn to recognize the OCDemon, who tries to trick you into turning small lapses into full scale relapse. Attendees will be interactively guided through the process of developing their own relapse prevention/recovery plan.

'Draw a Monster' Mindfulness and ACT Interactive Table
THE CHALLENGE: Draw a monster that represents an intrusive fear/thought or negative feeling associated with them. Then name it, draw goofy clothes on it, write down where you might take it if you just allowed yourself to shift your perspective and view it differently. THE GOAL: To view fear from a different lens. Once you stop wasting valuable energy on pushing your fears/monsters away, you can use it on the things in your life that hold true value and meaning. Have compassion for your thoughts, expand your mind to make room for your monsters. Once you give fear/monsters zero power, you will see that they will shrink and grow weak. Come to this activity to learn a fun way to stop fighting your fears/monsters, and change the way
you view them.

Mental Health Monologues: Sharing Stories, Reducing Stigma; and Performance by Comedian Dean Masello: "I'm So OCD"
Join us to develop an understanding of how to foster an open dialogue about mental illness within your community. We will discuss the process for producing a Mental Health Monologues performance, and how you can bring this experience to your own campus. We will present past performances from the University of Michigan’s 2015/2016 Mental Health Monologues Show and give you the opportunity to learn about an effective process for sharing personal stories in an impactful and understandable way. We are working to change the conversation
about mental health in our community, and hope to inspire you to recognize the ability for performance art to be an effective mode of expression for building greater understanding of mental health concerns. We will close with a performance by Dean Masello.

Dean Masello has had crippling anxiety for as long as he can remember — at least as far back as kindergarten. As a teenager, he came to believe that he was destined for psychosis. His one-man show “I’m So OCD”, is a comedic and harrowing ride into the dark terrors of the mind. Through storytelling and high-tech wizardry, Dean deftly illustrates the intensity of obsessive-compulsive disorder, and how the most dangerous traps are the walls we create within our own minds.

Sunrise Yoga: Flow into CBT

As OCD therapists, we're good at asking our clients to put themselves in unusual, sometimes uncomfortable, positions — usually in terms of how they think and behave. What if we could boost that by doing the same with their bodies? Learn about a novel approach to OCD treatment that incorporates yoga, which does more than just relax the body, it strengthens the brain's executive functioning skills (even more so than traditional physical activities). Get tips on how to incorporate yoga with your CBT/ERP practice and learn first-hand by participating in a mini-session led by a certified yoga instructor and LPC who will guide the group through yoga's three basic elements: Posture, breathing & meditation. Q&A opportunity to follow the practice.

YOGA MATS WILL BE PROVIDED (OR YOU CAN BRING YOUR OWN)!

OCD And Addiction Town Hall: Next Steps!
This year's Town Hall aims to build on last year's goal to bring this important issue into the light. Many attendees of that forum were able to talk openly about this often-secret issue for the first time. Now that we recognize the true scope of this problem, we must turn to “what can we do?”  We will discuss the good and bad of the present social and treatment landscape, and why integrating OCD and addiction treatment is so important to bring to the forefront of our advocacy efforts. Thinking Backwards: Overcoming OCD Through Paradoxical Intention

Heighten your anxiety in order to lower your anxiety. If you need to leave, you need to stay. Do on purpose what you are afraid you'll do by accident. Gain control by letting go of control. The understanding and treatment of OCD is full of counterintuitive concepts that are difficult for most people, at least initially, to understand and accept. This presentation reviews how “thinking backwards” not only makes sense in the treatment of OCD, but that it is the most powerful strategy for undermining the disorder and moving towards recovery. Multiple illustrations from actual OCD cases will be reviewed so that participants may clearly see specific applications in the real world of OCD.

CBT for OCD Characterized Primarily by Intrusive Thoughts, Using ERP and ACT Approaches

In the past, OCD characterized primarily by intrusive, unwanted, troubling thoughts of a harming, sexual, or religious/moral nature, with no observable rituals, was considered difficult or impossible to treat. Sufferers were often misdiagnosed, not properly treated, and left feeling tormented and ashamed. In fact, primarily obsessional OCD can be successfully treated. This workshop – geared toward mental health professionals but also valuable for individuals with OCD – will address using CBT, including Exposure and Response Prevention (ERP), and Acceptance and Commitment Therapy (ACT) approaches in treating primarily obsessional OCD. It will also include a roleplay demonstration using an imaginal exposure.

Step Back, Want It, Step Forward, Be Cunning: A Self-Help Paradigm

We should all begin addressing OCD with the least expensive and least time-consuming intervention, and progress to costlier treatment as needed. One initial step is a simple, active, paradoxical self-help protocol that lets patients generate their own set of homework assignments to challenge the dominance of the disorder moment-by-moment. The four components of this model will be presented. “Step Back” refers to the ability to gain perspective in the moment of distress. “Want It” emphasizes the intention to access an emotional state counter to fear. “Step Forward” implies the typical assignment of exposure, while adding this new point of view. “Be Cunning” suggests specific tactics in the moment of doubt and distress.

Multiple Perspectives on Taboo Intrusive Thoughts: Q&A With Former Sufferers

Taboo obsessions are called “taboo” for a reason – they embarrass those who suffer from them, causing them to hide their pain in shame, sometimes for decades. Their fear of being judged and ostracized keeps them silent, often until they finally understand that the thoughts can be treated under the diagnosis of OCD. Panel participants have all struggled with taboo thoughts and managed to largely overcome them – but not without some confusion and pitfalls along the way. They'll share their personal stories as well as their strategies for success, and will take questions from the audience.

Pharmacology Roundtable: The Pharmacologic Treatment of Resistant OCD and Related Disorders

The DSM V created a brand new category for diagnoses – OCD and Related Disorders. This category includes OCD, Body Dysmorphic Disorder (BDD), Hoarding Disorder, Hair Pulling Disorder (trichotillomania), and Skin Picking Disorder (excoriation). Each disorder shares the commonality of repetitive behaviors, though the specific nature of each disorder varies. What these disorders do share is a limited response to medication. Treatment resistant OCD and Related Disorders are almost the norm rather than the exception. While there is overlap in the pharmacologic treatment of these disorders, there are also important difference. The panel will discuss pharmacologic approaches to treatment resistance in all OCD and Related Disorders. This will be followed by audience Q&A.

General Session: Keynote Address & Awards
Keynote Address:

When David Adam published a book in 2014 about his experiences with OCD, he was not prepared for what came next. Hundreds of people with the condition got in touch, schools asked him to talk to their children, the International OCD Foundation gave him an award and his book was discussed by academics and used as an example of how a new generation is determined to challenge the stigma of mental illness. And all David thought he had done was, as a journalist, to try to tell an honest story. What does it say about the state of the media and public understanding that simple truths are heralded in this way? Join David for an illuminating and entertaining tale of his journey through OCD — from a patient to accidental advocate for others.

OCD and on the Autism Spectrum: Help, What Do We Do Now?
Patients with OCD who are also on the Autism Spectrum face unique difficulties in treatment that often necessitate altering traditional CBT methods and approaches. This talk will address the challenges, complications, and strategies of working with these individuals and their families. Case examples, role plays, and structured approaches will engage and encourage participants (clinicians, families, and patients) to think outside of the OCD box. Participants will learn ways to conceptualize and address functional problems and goals, develop techniques to improve anxiety and social skills, and learn to identify and address important treatment pitfalls and difficulties. By incorporating these elements into a CBT protocol, treatment can be tailored to address the complex array of symptoms often seen with these patients.

Saving Faith: Managing Religious Scrupulosity in the middle of exposure therapy
Religious scrupulosity poses subtle challenges for sufferers who wish to cultivate faith while undergoing treatment, especially when the therapist or patient misunderstand good mental health as necessitating abandonment of traditional religious values or practices. Scrupulosity is viewed as the enemy of faith versus the faith community. Participants will learn how a healthy faith and religious practice is founded upon the ability to tolerate doubt and uncertainty while living with hope. Ways to formulate and apply exposure with response prevention while respecting and adhering to traditional faith practices will be described. Ways to develop a standard for healthy faith and religious practice that adheres to traditional religious values will be described as it applies to Christian, Jewish and Muslim traditions.

From Research to Clinical Practice: Family-Based Treatment for Children and Adolescents with OCD
Clinical research and treatment guidelines have repeatedly recommend including family in the treatment of pediatric OCD. However, translating this into clinical practice is often met with barriers. Despite these guidelines, many children are still treated individually without family involvement. Considering the long-term effects of OCD, disseminating family-based treatment models will help to bridge the gap between research and clinical practice. This panel, comprised of leading experts in pediatric and family OCD research/treatment, will highlight the latest research and challenges in working with the family system in pediatric OCD treatment. Each panelist will illustrate a particular therapeutic approach, then engage the audience in discussion. This will be a highly interactive experience to reflect on clinical dilemmas, bringing together experienced researchers and clinicians.

Social Media and Infomania: Impulsive vs. Compulsive Behavior
Impulsivity is a pattern of acting without thinking, driven by pleasure-seeking behaviors and avoidance of pain that often results in negative consequences or undesirable outcomes. Compulsive behavior is driven by a need to rid oneself of doubt, anxiety, or uncertainty. Sometimes these behaviors are confused, and treatment for each is different. The focus of the workshop is to learn the similarities and differences between impulsive and compulsive behavior using the example of how people use (and misuse) social media.

Treatment of Obsessive Compulsive Disorder with Severe Psychiatric Comorbidities
OCD is a disorder marked by intrusive thoughts and repetitive behaviors aimed to reduce the negative affect associated with these thoughts. Although there is a large evidence-base for the use of cognitive-behavioral therapy with exposure and response prevention (ERP) and selective serotonin reuptake inhibitors (SSRIs) to treat OCD, various psychiatric comorbidities complicate the implementation of such standard interventions. The presenters will review OCD treatment and the complications that arise from psychiatric comorbidity – specifically, comorbid diagnoses of Bipolar Disorder, Autism Spectrum Disorder, and Tourette's Disorder will be discussed.

Making and Supporting Daily Disclosure Decisions
Many people with OCD and related disorders have experienced stigma in their everyday lives. This can affect whether a person feels safe or comfortable talking to friends, family, and their community. There is encouraging research indicating that sharing personal stories of recovery is effective in reducing self and public stigma. This workshop will explore the application of evidence-based frameworks for stigma reduction and disclosure decisions. A panel will share tips from personal and professional experience to use when faced with choices about if and what to say about their challenges and recovery journey in various settings. Strategies for individuals, advocates, and family members will be discussed, as well as how professionals can best support their clients in this process.

When Food Is The Enemy: ERP For A Variety Of Disordered Eating Habits
During this session, presenters who have worked with both OCD & eating disorders (ED) will share insights and case examples about how to use ERP to treat disordered eating, whether it's OCD with ritualized food/eating issues, sensory/spectrum-related rigid eating, “scrupulosity”-driven diets, or traditional eating disorder concerns.  Additionally, our nutritionist will discuss the dietary needs patients have and how nutritional planning would be applied depending on the diagnosis, as well as how nutritionists and therapists can work together to create separate but related food and non-food hierarchies. Though different disorders, both are treated by directly addressing the triggers and feared situations so that clients can move forward and learn to handle their discomfort, no “trigger warnings” necessary.
